FAMILY DRIVING(J FORMS)
Every year each member of your household that will be driving our NSISP students need to read and agree to the terms of the J form. To make it easier for you to complete, you can click the link below and fill in each section. This will notify the TCRCE NSISP that you have completed the form and will send you a copy completed. It is important to understand that the form gets you to type your name as an official declaration of your signature (so you do not need to print anything and sign). Each member must type their own name to agree. TCRCE Staff with NSISP will be witnessing the J forms being completed and a copy will be sent to you and the regional centre to be put on file.
